For $300 per year you will be
enrolled in a private contract with Hatteras Island Family
Medicine. This will allow you to be scheduled for routine
office
visits for a fee of only $20.00. An annual preventative exam
or
complete physical including screening pap smear is included in the HI
Access Card. Labwork includes annual screening labs and
followup
labs for diabetes, hyperlipidemia and hypertension. Other
non-routine labwork that is sent out is offered at our regular reduced
rate. Services like sutures or injections are not
covered
with the HI Access Card; however one annual flu shot is included.
This is a private one year contract
between Hatteras
Island Family Medicine and the patient and is nonrefundable.
Insurance will not reimburse for a prepaid plan and this is not a type
of insurance. It is simply a prepaid discount plan.
Some
insurance will reimburse patients directly for any claims they submit
for services such as office visits. Hatteras Island Family
Medicine makes no guarantees regarding reimbursement by insurance
companies and does not file insurance claims or sign insurance
contracts. Medicare and Medicaid patients are not allowed to
file
claims.
